NEED HELP -- Lost the fillable fields I added to work doc

Nitro Pro users, please help! My workplace just switched to Nitro Pro, and I am not yet familiar with the tool. My team asked me to edit a PDF (created in another application) to add fillable fields, and I spent a lot of time adding fields and trying to make them straight and aligned. Then I did something, and accidentally "hid" all my work! The form no longer looks fillable. I cannot figure out how to revert it. In order for me to further assist you with your concern, could you please provide the following?:
-What software was used to create the PDF and add fillable fields?
-The serial number of your Nitro Pro. To send it via private message, hover your mouse on top of my name 'AllainU' and select 'Message'. 
-Build of Nitro Pro are you working with. You can find this out by clicking on 'About Nitro Pro' under the Help tab (format is 13.X.X.X, e.g. 13.2.3.26) or you can go to Control Panel > Programs and Features.
-A copy of the original PDF file (with sensitive information extracted) and the resulting file (with the issue present) so I can replicate it on my end.
